Mucopolysaccharidosis type I (MPS I) is a rare lysosomal storage disorder treated with bone marrow transplantation or enzyme replacement therapy with laronidase, a high-cost orphan drug. Laronidase was approved by the US Food and Drug Administration and the European Medicines Agency in 2003 and by the Brazilian National Health Surveillance Agency in 2005. Many Brazilian MPS I patients have been receiving laronidase despite the absence of a governmental policy regulating access to the drug. Epidemiological and treatment data concerning MPS I (...) are scarce. This study aims to present a demographic profile of Brazilian patients with MPS I, describe the routes of access to laronidase in Brazil, and discuss associated ethical issues relating to public funding of orphan drugs. The ongoing coronavirus disease 2019 (COVID-19) pandemic has quickly swept the globe leaving a devastating trail of lost human lives and leading to a public health and economic crisis. With this in mind, prosociality has been heralded as a potential important factor to overcome the negative effects of the pandemic. As such, in this study, we examined the effectiveness of a brief reflexive writing exercise about recent experiences of gratitude on individuals’ intentions to engage in prosocial behaviors using a sample (...) of 253 participants living in Portugal and 280 participants living in Brazil. Participants were randomly assigned to either a condition in which they were asked to write about recent experiences of gratitude or a control group in which they were asked to write about daily tasks. We predicted that the gratitude intervention would increase state gratitude and, consequently, increase positive affect and empathic concern, and decrease negative affect, leading to increased intentions to engage in prosocial behaviors during the COVID-19 pandemic. A moderated serial–parallel mediation analysis, in which we controlled for gender, age, and level of religiosity, indicated that our manipulation led to increases in state gratitude, which in turn increased positive emotions and empathic concern, leading to increased prosocial intentions in both countries. A content analysis of participants’ responses in the gratitude group revealed that relationships with others and health and well-being were the central themes of their gratitude experiences during the COVID-19 global pandemic. In the Brazilian Amazon, snakebite envenomations (SBEs) disproportionately affect Indigenous populations, and have a significantly higher incidence and lethality than in non‐Indigenous populations. This qualitative study describes the Indigenous and biomedical healthcare domains for SBE care from the perspective of the Indigenous medical and nursing students in Manaus, Western Brazilian Amazon. In‐depth interviews were conducted with five Indigenous students from the Amazonas State University, between January and December 2021. The interviews were analyzed using inductive content analysis. We organized an explanatory (...) model with five themes: (1) participants' identities; (2) causality levels in Indigenous and biomedical systems; (3) therapeutic itineraries in Indigenous and biomedical systems; (4) ideological implications of adding biomedical devices to Indigenous healing systems; and (5) therapeutic failure in and efficacy of Indigenous and biomedical systems. From a noncolonial perspective and seeking to increase the quality and acceptability of health care for the Indigenous populations of the Brazilian Amazon, the training of Indigenous health professionals presents itself as a promising strategy. For this goal, universities should serve as empowering settings for Indigenous health students that support them in their growth and development, raise their awareness of injustice, and catalyze change toward a culturally adapted and effective service for the users. Placebo-controlled trials are controversial when individuals might be denied existing beneficial medical interventions. In the case of malaria, most patients die in rural villages without healthcare facilities. An artesunate suppository that can be given by minimally skilled persons might be of value when patients suddenly become too ill for oral treatment but are several hours from a facility that can give injectable treatment for severe disease. In such situations, by default, no treatment is (or can be) given until the patient (...) reaches a facility, making the placebo control design clinically relevant; alternative bioequivalence designs at the facility would misrepresent reality and risk incorrect conclusions. We describe the ethical issues underpinning a placebo-controlled trial in severe malaria. To protect patients and minimise risk, all patients were referred immediately to hospital so that each had a higher chance of prompt treatment through participation. There was no difference between artesunate and placebo in patients who reached clinic rapidly; among those who could not, a single artesunate suppository significantly reduced death or permanent disability, a finding of direct and indirect benefit to patients in participating villages and elsewhere. Neurosurgery for psychiatric disorders, also sometimes referred to as psychosurgery, is rapidly evolving, with new techniques and indications being investigated actively. Many within the field have suggested that some form of guidelines or regulations are needed to help ensure that a promising field develops safely. Multiple countries have enacted specific laws regulating NPD. This article reviews NPD-specific laws drawn from North and South America, Asia and Europe, in order to identify the typical form and contents of these laws and to (...) set the groundwork for the design of an optimal regulation for the field. Key challenges for this design that are revealed by the review are how to define the scope of the law, what types of regulations are required, and how to approach international harmonization given the potential migration of researchers and patients. BackgroundThis pilot study aims to test the usability of the iACTwithPain platform, an online ACT-based intervention for people with chronic pain, to obtain information on which intervention and usability aspects need improvement and on expected retention rates.MethodsSeventy-three Portuguese women with chronic pain were invited to complete the first three sessions of the iACTwithPain intervention assess their quality, usefulness and the platform’s usability. Twenty-one accepted the invitation. Additionally, eight healthcare professionals working with chronic medical conditions assessed the platform and the intervention (...) from a practitioner’s point of view.ResultsThis study presented a considerable attrition rate (71.43%) among chronic pain participants, with six completers. There were no significant differences in demographic or clinical variables between dropouts and completers except for completed education (participants who dropped out presented less education than completers). Reasons for dropout were related to difficult personal events occurring during the time of the intervention, lack of time, or having forgotten. There seemed to be an overall satisfaction with both the intervention, its contents and form of presentation of information, and the platform, concerning its design, appearance, and usability. Real image videos were preferred over animations or audio by chronic pain participants. Healthcare professionals emphasized the appealing and dynamic aspects of the animation format.ConclusionThis study informs the ongoing improvement of the iACTwithPain platform and provides valuable information on aspects researchers should consider while developing online psychological interventions for chronic pain. Heterogeneity within MDD has hampered identification of biological markers (e.g., intermediate phenotypes, IPs) that might increase risk for the disorder or reflect closer links to the genes underlying the disease process. The newer characterizations of dimensions of MDD within Research Domain Criteria (RDoC) domains may align well with the goal of defining IPs. We compare a sample of 25 individuals with MDD compared to 29 age and education matched controls in multimodal assessment. The multimodal RDoC assessment included the primary IP (...) biomarker, positron emission tomography (PET) with a selective radiotracer for 5-HT1A ([11C]WAY-100635), as well as event-related functional MRI with a Go/No-go task targeting the Cognitive Control network, neuropsychological assessment of affective perception, negative memory bias and Cognitive Control domains. There was also an exploratory genetic analysis with the serotonin transporter (5-HTTLPR) and monamine oxidase A (MAO-A) genes. In regression analyses, lower 5-HT1A binding potential (BP) in the MDD group was related to diminished engagement of the Cognitive Control network, slowed resolution of interfering cognitive stimuli, one element of Cognitive Control. In contrast, higher/normative levels of 5-HT1A BP in MDD (only) was related to a substantial memory bias toward negative information, but intact resolution of interfering cognitive stimuli and greater engagement of Cognitive Control circuitry. The serotonin transporter risk allele was associated with lower 1a BP and the corresponding imaging and cognitive IPs in MDD. Lowered 5HT 1a BP was present in half of the MDD group relative to the control group. Lowered 5HT 1a BP may represent a subtype including decreased engagement of Cognitive Control network and impaired resolution of interfering cognitive stimuli. Future investigations might link lowered 1a BP to neurobiological pathways and markers, as well as probing subtype-specific treatment targets. (shrink)

Background: Chronic Pain (CP) has serious medical and social consequences, and leads to economic burden that threatens the sustainability of healthcare services. Thus, optimized management of pain tools to support CP patients in adjusting to their condition and improving quality of life is timely. Although Acceptance and Commitment Therapy (ACT) is considered an evidence-based psychological approach for CP, evidence for the efficacy of online-delivered ACT for CP is still scarce. At the same time, studies suggest that self-compassion mediates the change (...) in disability and psychopathological symptoms in ACT interventions for CP, although self-compassion is not a specific target in ACT. Thus, an explicit focus on self-compassion might increase the efficacy of ACT interventions for CP, although this hypothesis has not been tested. This study aims to develop an eHealth ACT and compassion-based self-management intervention for CP, the iACTwithPain, and to compare its efficacy in improving health outcomes to a similar ACT-only intervention and a waiting list group. Methods: The eHealth platform that will host the interventions will be developed using a flat design identity and will be interactive. The iACTwithPain intervention will comprise 8 weekly self-management sessions, and will be developed taking into consideration the psychological flexibility model applied to CP, with the addition of explicit compassion-based components. To analyse whether the iACTwithPain intervention will present superiority in improving CP’s impact and related health markers over the two other conditions, this study will follow a RCT design with three arms. CP patients will be recruited through direct contact with patient associations and healthcare services and a national press release in Portugal. Outcome measurement will be conducted at baseline, post-intervention and at 3- and 6-months follow-ups. The interventions’ acceptability will also be assessed. Discussion: The iACTwithPain intervention is expected to improve CP patients’ psychosocial functioning, quality of life, and empowerment, by promoting adaptive disease management and regulation of pain-related internal experiences. Results will contribute to a better understanding on the pertinence of adding compassion elements to ACT for CP and to reach an optimized intervention for CP. (shrink)

Our starting point is a remark made by an art historian, Charles Carman, to the theorist of art Norman Bryson regarding his interpretation of the Japanese philosopher Nishitani Keiji's standpoint of emptiness. Bryson claims that Nishitani's standpoint supports his contrast between Western and non-Western gaze. On the contrary, Carman sees Nishitani's standpoint closer to the Renaissance theory of vision as found in Alberti and Nicholas von Cues and so not as opposite to Western tradition as a whole but to a (...) major trend of it. Our suggestion here is that while Nishitani's standpoint illustrates an Asian way of thinking, at the same time it sheds light upon Western assumptions. Therefore, it constitutes a demonstration of how much it can be significant to integrate non-European cultures into the philosophical and art theory discussions in order to broaden our understanding of the discipline of aesthetics. Pretendo neste artigo discutir alguns usos singulares que os pensadores Gilles Deleuze e Félix Guattari, em O Anti-Édipo, fazem da “segunda dissertação” da Genealogia da Moral de Friedrich Nietzsche no intuito de formular uma economia política das sociedades primitivas através do problema da dívida e da crueldade intimamente ligada. Gostaria também de minimamente apresentar como essas práticas foram essenciais para esconjurar a formação do Estado.

Este artigo tem como objetivo ampliar a visão do fazer científico, abordando a análise feita por um dos maiores epistemólogos contemporâneos da natureza do fazer científico e de sua dinâmica, de modo a contribuir para a redução do fosso existente na sociedade científico-tecnológica entre a visão dos problemas fundamentais do homem e a compreensão da ciência e de sua tecnologia. Assim sendo, o nosso estudo se restringe ao modo de ver de Karl Popper o lado mais interno da ciência, evidenciando (...) igualmente a sua estrutura na ética que suas conquistas tecnológico-instrumentais. A questão a ser tratada é como o autor enfrenta o progresso na ciência. Mais especificamente, a ciência progride? (shrink)

Na filosofia de Platão e do platônico Aristóteles há muitas coisas imperfeitas: devir, multiplicidade infinita, indeterminação, contradição, efemeridade, acaso, necessidade cega, acidente, mistura, potência, matéria, não-ser, mal etc. Não houvesse, o sistema não giraria bem em seus gonzos e não daria conta do projeto epistêmico de dizer o real tal como é. Mas há em compensação um fator de perfeição que organiza e subordina tudo isso: a primazia da forma (eîdos). Nenhuma daquelas noções existe sem mais, mas orbita em torno (...) de algum item ligado ao campo semântico da forma: subsistência, unidade, identidade, determinação, teleologia etc. Desse modo, ainda que se admita serem os primeiros itens tão originários quanto os segundos, dá-se uma hierarquia de caráter axiológico, numa assimetria ontológica de um outro jogo de anterioridade e posterioridade. Arguir contra a primazia da forma equivale a arguir a favor da primazia da matéria. Que paisagem se contempla quando se encara o real pela perspectiva de uma filosofia de tipo pré-socrático, marcada agora não pelo signo da insuficiência, mas, ao contrário, como filosofia plena? E se a segunda navegação do Fédon devesse retornar à primeira? E se a filosofia resumida pelo Estrangeiro de Atenas em Leis 888e4-890a1 merecesse não a censura, mas o aplauso? (shrink)

Argumentation mining aims to automatically detect, classify and structure argumentation in text. Therefore, argumentation mining is an important part of a complete argumentation analyisis, i.e. understanding the content of serial arguments, their linguistic structure, the relationship between the preceding and following arguments, recognizing the underlying conceptual beliefs, and understanding within the comprehensive coherence of the specific topic. We present different methods to aid argumentation mining, starting with plain argumentation detection and moving forward to a more structural analysis of the detected (...) argumentation. Different state-of-the-art techniques on machine learning and context free grammars are applied to solve the challenges of argumentation mining. We also highlight fundamental questions found during our research and analyse different issues for future research on argumentation mining. (shrink)

Resenha do livro: MACINTYRE, Alasdair. Deus, a Filosofia e as Universidades: Uma história seletiva da Tradição Filosófica Católica. Brasília: Editora Devenir, 2022.

Brazil has one of the highest levels of economic disparity in the world. The educational system plays a large role in this reality, acting as a mechanism of social exclusion. Neoliberalism has resulted in the commodification of education, empowering private schools while undermining the public system. This has created a vicious cycle, whereby educational inequality reflects and reinforces social inequality. Such a system violates the rights of children not lucky enough to be born into wealth – the right to equal (...) education; to equal opportunity; and equal treatment. From within this context, we propose a model, dubbed The Eagle’s Flight, for psychological intervention in public schools. This will form an extension of our research group’s Psychosocial Assessment and Intervention – Prevention, Community and Liberation, which has been an on-going project for the past fourteen years. The intervention model advocates monitoring child development from a critical, social and historical perspective, focusing on how school and community affect everyday life. The analysis will be carried out via the daily immersion of psychologists in the public school setting, who pay attention to key aspects which include various expressions of violence, financial difficulties related to unemployment, informal employment or drug dealing, poor access to health services, and the developmental impact of factors such as poverty. Based on Paulo Freire's Emancipatory Education Proposal and Ignacio Martín-Baró’s Liberation Psychology, intervention in school and community must be an interdisciplinary procedure, employing professionals from a range of disciplines involved in the study of child development. The various subjects’ perspectives seek to provide support for everyday problems and solutions, whilst breaking down the hegemonic model of psychological practice that considers the child and family as responsible for their problems. A concrete example of this critical psychosocial intervention model is presented. (shrink)

Este reciente libro de la filósofa Sharon Krishek aborda de forma profunda y exhaustiva el tema del amor (en especial, el amor romántico) en el contexto de los planteamientos que Kierkegaard elaboró en sus obras acerca de esta importantísima dimensión de la existencia humana. El proyecto del libro es definido por Krishek, valientemente, como “neo-kierkegaardiano”; este apelativo responde con cuidado a su propósito: si bien los planteamien- tos y nociones del filósofo danés son la base y el motor de la (...) reflexión, los desarrollos y conclusiones a los que llega la autora exceden por mucho la filosofía de Kierkegaard e, incluso, en ocasiones, llegan a señalar sus deficiencias o ambigüedades.
